If you wish to have records from your previous dentist released to our office,  please print and complete the following form.  Send the Release of Records form directly to your previous dentist. If you prefer you can mail, fax or email the completed form to our office and we will gladly assist you in retreiving your records.

Please remember that if you wish to have us receive your records before your first appointment,please send the form no later than 2 weeks before your scheduled appointment.

Records are not necessary for your first visit.
